Short-Term and Long-Term Disability Insurance

SHORT-TERM DISABILITY INSURANCE

Short-Term Disability (STD) insurance provides benefits-eligible and union employees with a benefit of 70% of base salary for each day they are absent due to illness or injury, up to a maximum of 26 weeks in a period of 12 consecutive months. All active full-time employees are eligible for this plan, unless employed in a state which provides state-covered disability insurance. For more information about this benefit, refer to your Leave of Absence policy, available on the HR Policies section of the Sample Company intranet, or contact your HR representative or the Benefits Department.

LONG-TERM DISABILITY INSURANCE

Long-Term Disability (LTD) insurance through Unum is designed to replace a portion of your income if you become totally disabled and cannot work for an extended period of time. LTD benefits begin after you have been totally disabled for at least 180 consecutive days.

Eligibility

Non-union employees in the U.S. who work at least 30 hours per week are automatically enrolled in LTD insurance effective on their first day of employment.

The benefit is equal to 66 2/3% of your base salary. A monthly maximum benefit will apply. Payment of the benefit is subject to approval by the insurance company.

Long-Term Care Insurance

Long-Term Care insurance is an additional insurance program offered to our team members so they have the convenience of payroll deduction and discounted group premiums for other coverage required to support their individual and family needs. Long-Term Care (LTC) provides services which help meet both the medical and non-medical needs of people with a chronic illness or disability who cannot care for themselves for long periods.

It is common for Long-Term Care (LTC) to provide custodial care such as assisting with normal daily tasks like dressing, feeding, bathing, using the bathroom. LTC also provides a level of care that requires the expertise of skilled practitioners who can handle multiple chronic conditions.

  • This policy is owned by the employee and the employee pays 100% of the cost of coverage
  • Employees will have 31 days from the date of hire to request an application for enrollment in this policy through the Benefits Department. Once this 31-day window is missed, the only opportunity to enroll will be during future voluntary program open enrollments which are held periodically
  • Employees may discontinue this policy at any time upon notification to the insurance company. To enroll in Long-Term Care, use the step by step instructions provided in this link: example.com
  • This policy is also portable, meaning that if employment is terminated for any reason, upon termination employees have the right to take the coverage as an individual policy and continue to pay premiums directly to Unum
